Admitting
A case coordinator will be assigned to you, the patient, and his or her role is to assist with any discharge planning needs you may have. He or she will met with you within 48 hours of your admission.

Going Home
You may require skilled healthcare when you are discharged from the hospital. That care may be provided at a rehabilitation facility or at home with nursing services from a home care agency.
Advance Directives
You have the right to make health care decisions about the medical care you receive. If you do not want certain treatments, you have the right to tell your physician you do not want them and have your wishes followed.
As a courtesy to our patients, Hartford Hospital submits bills to your insurance company and will do everything possible to advance your claim.
Bloodless Medicine
Through a multidisciplinary approach to patient care, complex procedures such as cardiac surgery, including off-bypass procedures, aortic aneurysm repair, total joint replacements and kidney transplants are performed by physician’s who specialize in techniques of non-blood management.

Your caregivers will use a 0-10 Pain Scale to help us assess your pain and response to pain medicines. Aside from the expertise provided by your caregivers, there are different pain control consultative services that are available to patients when necessary.
